Dear Commissioner Mullen:

Re (CRAHD) Connecticut River Area Health District's form Exhibit A, "APPLICATION FOR INITIAL INVESTIGATION."
The Exhibit A is not a form that requires approval by the CT DPH, and therefore no approval letter from DPH has been requested or received. The forms referred to in Section 19-13-B103 of the CT Public Health Code are limited to forms used specifically for permit applications, site investigations and installation inspections of on-site sewage disposal systems. - Jim Monopoli 29Apr13
Exhibit A is labeled an "INVESTIGATION," and Monopoli cites Section 19-13-B103 as covering investigations.

What's special about Monopoli's Exhibit A "investigation," that it doesn't require approval by DPH?

What am I missing here?
