Dear Mr. Anderson:

This letter is in response to your April 30, 2013 correspondence to Commissioner Jewel Mullen, MD, MPH, MPA concerning an application you received from the Connecticut River Area Health District (CRAHD). As you are aware, the Department responded to your inquiry in March and April 2013. To reiterate, the form does not requires approval from the Department and is specific to the Town of Old Saybrook's Decentralized Wastewater Management District (DWMD).

The CRAHD correspondence makes reference the Town of Old Saybrook's DWMD ordinance, and the Old Saybrook Water Pollution Control Authority (WPCA). If you have other questions concerning the Town of Old Saybrook's DWMD, we recommend that you direct them to CRAHD or the Town of Old Saybrook WPCA.
